If the buyer is a trust, the buyer must provide the trust’s legal name, structure, and address, as well as personal identifying information for relevant individuals connected to the trust, such as trustees, certain grantors or settlors, and beneficiaries with control or withdrawal rights. The same personal information is required for each such individual, and the trust may also need to disclose whether it is revocable and who has authority to act on its behalf.

In all reportable transactions, buyers may also be asked to identify who is signing documents on behalf of the trust and provide the same level of personal information.  All such parties must also provide a certification confirming that the information provided is accurate.
