If the buyer is a legal entity such as an LLC, corporation, or partnership, the buyer must provide the entity’s legal name, type, jurisdiction of formation, and address, along with personal identifying information for all beneficial owners. Beneficial owners include any natural person who owns or controls 25 percent or more of the entity and other individuals who exercise substantial control, and the required personal information includes name, date of birth, residential address, citizenship, and taxpayer identification number such as a social security number, TIN or ITIN.

In all reportable transactions, buyers may also be asked to identify who is signing documents on behalf of the entity.  If such person separate from a beneficial owner, the same personal information that is required for a beneficial owner is required to be disclosed by the person exercising control over the entity. Each person providing such information must also provide a certification confirming that the information provided is accurate.
